By registering on the HMS Patient Portal, you consent to the collection, storage, and processing of your personal and health-related information. This data is used solely for the purpose of providing medical care, coordinating treatment, managing appointments, and communicating health information to you.
Appointments must be cancelled or rescheduled at least 24 hours in advance. Late cancellations (within 24 hours) are subject to a ₱560.00 administrative fee. Patients who fail to attend a scheduled appointment without prior notice will be charged a ₱1,500.00 no-show penalty. Three (3) no-show incidents may result in temporary suspension of online booking privileges.
Your health records are protected under applicable medical privacy laws. Your data will not be shared with third parties without your explicit written consent, except where required by law or for direct care coordination.
You are responsible for maintaining the confidentiality of your portal credentials. Unauthorized use of your account must be reported immediately. The hospital reserves the right to suspend accounts engaged in misuse.
If you use telehealth services, you consent to electronic communication of health information and acknowledge that technical limitations may affect service quality. Telehealth services do not replace in-person emergency care.
